Description: Downy brome or cheatgrass is an annual or winter annual introduced from Eurasia that reproduces by seed. The plants are 4 to 30 inches tall and are covered with soft hairs at all stages of growth. Mature cheatgrass seedheads, 2 to 6 inches long, contain seed, 3 8 to 3 inch long. The awns are usually purplish at maturity. Cheatgrass competes with more desirable perennial grasses for moisture because of its winter and early spring growth habit. After maturity it becomes a nuisance and fire hazard. Comments: Under field conditions, downy brome seeds germinate in fall, winter and spring. Most seeds germinate within one year of maturation. Constant late summer or late fall rains will cause rapid germination and abundant fall growth. However, if adequate fall moisture is not available, downy brome can act like a spring annual. Biological control: Scientists are working on specific bacteria for biological control, but this is still at the testing stage. Chemical control: Several herbicides are effective at label rates. The most important factor to consider when initiating a control program is timing. This weed must be controlled early in the season, prior to seed set, or late fall. Cultural control: Mowing or cultivation effectiveness will be increased if other plants like perennial native grasses or alfalfa are seeded in the downy brome stands as competitors. Maintain range and pasture in good condition. Promote healthy grass growth through proper irrigation and fertilization. Do not overgraze. Mechanical control: Neither mowing nor burning are effective controls to prevent seed set. Seed will continue to mature on cut-off plants. One option is mowing and removing plants from the field. Burning is not effective, as many of the seeds drop to the ground before the heat can kill them. Early spring tillage of fallow ground, tillage prior to planting spring-season crops, and tillage operations that bury downy brome seeds mold-board plowing ; are effective mechanical methods. Are first evaluated at the Risk Reduction Clinic, they report eating very little, yet fail to lose weight. While a majority may eat smaller-than-average meals, they are choosing high-calorie items with low nutritional content." He advises physicians whose patients attempt lifestyle changes to offer consistent support, but also request accountability. "Ask patients to record everything they eat and every physical activity they perform, and take a moment at their follow-up visit to review the log. Encouraging people to self-monitor is the first step in making them self-aware." Patients who are newly diagnosed with hypertension, diabetes, or other conditions that demand lifestyle changes affecting an entire family may benefit from early referral to a nutritionist. However, Ard says, those who have been effectively managed with drugs and want to attempt lifestyle changes to avoid additional medications may benefit more from their primary physician's continuous support. Once lifestyle modifications become routine, less intense monitoring is required, but physicians should continue to encourage patients to record activities and focus on overall health more than immediate results, such as weight loss. "Maintaining weight is as important, and potentially easier to focus on, than losing weight, " Ard says. PREMIER data showed 75% of the 5% to 6% weight loss from baseline to 6 months was preserved at 18 months. "These modest improvements should be viewed in the context of public health goals that emphasize preventing weight gain and increasing healthy food choices, " he says. "Participants who followed DASH ate more fiber and less saturated fat, reduced sodium intake, and boosted their nutrition. Future data will help further define predictors of success." The NHLBI Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure JNC 7 ; revised clinical guidelines in 2003, emphasizing the benefits of DASH and combining lifestyle changes, but suggesting most patients would require.

Mbracing the role of various imaging techniques has become paramount for cardiologists. With this responsibility comes the need to be able to recognize the strengths and weaknesses of cardiac CT and nuclear imaging for the diagnosis of chronic coronary artery disease. In an ACC.06 Spotlight Session, presenters examined the efficacy of these technologies in patients with chronic CAD. With regard to myocardial perfusion imaging in asymptomatic patients with risk factors, H. William Strauss, M.D., noted that guidelines stipulate the following: assess the presence, location, extent and severity of the myocardial perfusion abnormalities; determine the significance of anatomic lesions detected by angiography; and detect viable ischemic myocardium. With newer technologies available, cardiologists are gaining greater precision, but he added that some finesse is necessary with SPECT. "You have to apply the attenuation correction, " said Dr. Strauss, clinical director of nuclear medicine at Memorial-Sloan Kettering Cancer Center, New York. In symptomatic patients with an intermediate likelihood of coronary artery disease, Joao A. Lima, M.D., F.A.C.C., said that the data suggest that there could be a role for CT angiography in patients with low or intermediate risk for acute coronary symptoms. "This technology may be quite good at ruling out the presence of coronary artery disease. Patients who are negative by CT could be discharged, and those who are positive could go for further testing, " said Dr. Lima, associate professor of medicine and radiology and director of cardiovascular imaging at Johns Hopkins School of Medicine, Baltimore, Md. Dr. Lima thinks that cardiologists should: Remind each patient that exercise alone may be helpful Assess whether the patient has CAD, assess the patient's symptoms Share the risk of a cardiac event with the patient Assist physicians in making decisions. Comparison of Three Burrowing Crayfish Capture Methods and the Influence of Species, Morphology, Seasonal Activity, and Habitat Quality. Justin Ridge. Hanover College, Hanover, IN. Sponsor: Daryl Karns I compared three methods of capturing terrestrial burrowing crayfish. The methods included burrow excavation, the Norrocky burrowing crayfish trap, and the burrowing crayfish net. I sampled thirty stream banks within and around Big Oaks National Wildlife Refuge, in southeastern Indiana, from mid-August to mid-October. At each site, each method was equally replicated, ranging from five to ten replicates depending on burrow availability. I determined a measurement of trap efficacy by excavating the burrows after trap collection. I used species, habitat quality, and morphometrics to assess alternative factors that may influence capture success and crayfish distribution. Burrow excavation caught significantly more crayfish than the other two methods; the Norrocky burrowing crayfish trap was not significantly different from the burrowing crayfish net in trap efficacy. I found positive correlations between habitat quality and Norrocky burrowing crayfish trap and burrowing crayfish net success. The two species, Cambarus Tubericambarus ; polychromatus and Cambarus Lacunicambarus ; species " A", were significantly different in size, and I found there was a positive correlation between C. polychromatus size and habitat quality and cymbalta. The Latest Clinical Considerations Because antipsychotic therapy is a long term proposition in most patients, the clinician must carefully weigh the risks of medication adverse events, including metabolic and cardiovascular disorders. Manufacturers of clozapine, risperidone, olanzapine, and aripiprazole have modified prescribing information, advising prescribers of antipsychotic-associated cases of hyperglycemia, ketoacidosis, hyperosmolar coma, or death. Clinical experience and published literature indicate that clozapine and olanzapine are associated with the highest risk of weight gain, diabetes, and worsening lipid profile. The metabolic risk conferred by risperidone and quetiapine seems to be more contradictory; however, these two agents appear less associated with weight gain. The newest agents in the class, aripiprazole Abilify ; and ziprasidone Geodon ; are linked with the lowest risk of weight gain and do not appear to adversely impact the risk of diabetes or dyslipidemia. Interestingly, ziprasidone may have a positive effect on lipid profiles. On the down side, ziprasidone has been most associated with risk of prolonged QTc interval on the ECG, which may present a clinically significant risk in selected patients. The significance of the metabolic side effects of these newer antipsychotic agents has prompted a proactive approach to monitoring. The American Diabetes Association has developed a monitoring protocol for patients prescribed atypical antipsychotics. These recommendations include a baseline measurement of weight and body mass index BMI ; , waist circumference, blood pressure, fasting plasma glucose, and fasting lipids on all patients initiating therapy with an atypical antipsychotic. These recommendations may also apply for patients who are switching from one atypical agent to another. Weight should be monitored every four weeks for the first three months of therapy and quarterly thereafter. Waist circumference should be monitored annually. Upon baseline assessment, fasting plasma glucose and blood pressure should be monitored every 12 weeks and then annually. A fasting lipid profile should be obtained 12 weeks after beginning therapy and then at least every 5 years. In the case of pre-existing conditions such as hypertension, diabetes, or hyperlipidemia, it is clinically prudent to make patient-specific modifications to these monitoring guidelines. The most serious considerations are the potentially serious cerebrovascular side effects reported with two atypical antipsychotic agents, risperidone and olanzapine. Specifically, these agents are associated with a significant risk of a cerebrovascular accident occurring in elderly patients being treated with antipsychotics for psychiatric and behavioral disturbances related to dementia. Pooled data from 4 placebo-controlled trials in more than 1, 200 patients with Alzheimer's Disease or vascular dementia revealed that cerebrovascular adverse events were twice as common in risperidone-treated patients 4% ; as in the placebo-treated patients. While these antipsychotics can be beneficial in the treatment of psychosis, agitation, and behavioral disturbances associated with dementia, their use in this population represents off-label prescribing. In the face of these manufacturers' warnings, the Pharmacy and Therapeutics Committee of SHS has implemented restrictions for inpatient prescribing of risperidone and olanzapine. Therapy with either of these two agents in a patient over the age of 70 years may only be initiated by psychiatry or geriatric medicine. For elderly patients who have been taking these respective agents prior to hospital admission, the pharmacist will leave a note affixed to the front of the chart highlighting the potential risk. It will be left to the clinical discretion of the prescribing physician to weigh the risk vs. benefit when deciding to continue or modify the antipsychotic regimen. References and sinequan.

Research has shown that people with bipolar disorder are at risk of switching into mania or hypomania, or of developing rapid cycling, during treatment with antidepressant medication.15 Therefore, "mood-stabilizing" medications generally are required, alone or in combination with antidepressants, to protect people with bipolar disorder from this switch. Lithium and valproate are the most commonly used mood-stabilizing drugs today. However, research studies continue to evaluate the potential mood-stabilizing effects of newer medications. Atypical antipsychotic medications, including clozapine Clozaril ; , olanzapine Zyprexa ; , risperidone Risperdal ; , quetiapine Seroquel ; , and ziprasidone Geodon ; , are being studied as possible treatments for and pamelor. Medications are often used to treat behavioral problems, such as aggression, selfinjurious behavior, and severe tantrums, that keep the person with ASD from functioning more effectively at home or school. The medications used are those that have been developed to treat similar symptoms in other disorders. Many of these medications are prescribed "off-label." This means they have not been officially approved by the FDA for use in children, but the doctor prescribes the medications if he or she feels they are appropriate for your child. Further research needs to be done to ensure not only the efficacy but the safety of psychotropic agents used in the treatment of children and adolescents. A child with ASD may not respond in the same way to medications as typically developing children. It is important that parents work with a doctor who has experience with children with autism. A child should be monitored closely while taking a medication. The doctor will prescribe the lowest dose possible to be effective. Ask the doctor about any side effects the medication may have and keep a record of how the child responds to the medication. It will be helpful to read the "patient insert" that comes with the child's medication. Some people keep the patient inserts in a small notebook to be used as a reference. This is most useful when several medications are prescribed. Anxiety and depression. The selective serotonin reuptake inhibitors SSRI's ; are the medications most often prescribed for symptoms of anxiety, depression, and or obsessive-compulsive disorder OCD ; . Only one of the SSRI's, fluoxetine, Prozac ; has been approved by the FDA for both OCD and depression in children age 7 and older. Three that have been approved for OCD are fluvoxamine Luvox ; , age 8 and older; sertraline Zoloft ; , age 6 and older; and clomipramine Anafranil ; , age 10 and older. Treatment with these medications can be associated with decreased frequency of repetitive, ritualistic behavior and improvements in eye contact and social contacts. The FDA is studying and analyzing data to better understand how to use the SSRI's safely, effectively, and at the lowest dose possible. Behavioral problems. Antipsychotic medications have been used to treat severe behavioral problems. These medications work by reducing the activity in the brain of the neurotransmitter dopamine. Among the older, typical antipsychotics, such as haloperidol Haldol ; , thioridazine, fluphenazine, and chlorpromazine, haloperidol was found in more than one study to be more effective than a placebo in treating serious behavioral problems. However, haloperidol, while helpful for reducing symptoms of aggression, can also have adverse side effects, such as sedation, muscle stiffness, and abnormal movements. Placebo-controlled studies of the newer "atypical" antipsychotics are being conducted on children with autism. The first such study, conducted by the NIMH-supported Research Units on Pediatric Psychopharmacology RUPP ; Autism Network, was on risperidone Risperdal ; . Results of the 8-week study were reported in 2002 and showed that risperidone was effective and well tolerated for the treatment of severe behavioral problems in children with autism. The most common side effects were increased appetite, weight gain and sedation. Further long-term studies are needed to determine any long-term side effects. Other atypical antipsychotics that have been studied recently with encouraging results are olanzapine Zyprexa ; and ziprasidone Geodon ; . Ziprasidone has not been associated with significant weight gain and glyset and Order geodon online.

The Sub-Group is focusing on the template for hospital prescribing and pursuing work on developing the evidence base for the guidelines. 34. The evaluation of clinical pharmacist programme would be presented at the "Resistance to Change" conference to be held on 27th October 2006 and funded by DH. 35. It was noted that the Health Care Commission HCC ; Indicators for annual health checks would go public in September 2006. 36. Difficulties with denominator data were noted in the Outcomes of Antimicrobial Usage project using IMS data which links antimicrobial usage to MRSA and C.difficile rates. However, the committee were encouraged by the findings and potential of this data to provide useful information. Agenda item 7.3 Professional Education Sub-Group SACAR 16 9 + 37. The SWOT analysis highlighting the interest and enthusiasm of the stakeholders who attended the workshop were presented. The notion of bringing in competency standards could be seen as a threat and that getting agreement across professions and administrations was not easy. 38. Prescribing needs to be informed by data on local resistance patterns. A revised initiative giving clarity and consistency avoiding duplication and conflicting statements was proposed. This study showed an increase in serum phospholipids in rats treated with ISO. Increased peroxidation of membrane phospholipids releases free fatty acids via phospholipase A2 23 ; . Free fatty acids are substances for microsomal lipid peroxidation. The higher levels of serum free fatty acids in animals treated with ISO is due to increased lipolysis 24 ; . Paritha and Devi have also reported an increase in serum phospholipids in ISO-treated rats 20 ; . Hypertriglyceridaemia and increased levels of cholesterol ester in serum might be responsible for altered cardiovascular functions 25 ; . Hypertriglyceridaemia observed in ISO-treated rats is due to a decrease in the activity of lipoprotein lipase in the heart, resulting in decreased uptake of triglycerides from the circulation 22 ; . Pre-treatment with AMLEt in ISO-treated rats showed significant effect on lipids. Hence, AMLEt offers protection against myocardial infarction induced by ISO. This might be due to effective quenching of free radicals. This prevents activation of the lipid peroxidation. -tocopherol pre-treatment also showed a significant effect on lipids and lipoproteins. It has been reported that -tocopherol protects against LDL oxidation, thereby inhibiting the development of atherosclerosis 20 ; . Administered at a dose of 200 mg kg, AMLEt was more effective than a dose of 100 mg kg. Both AMLEt 200 mg kg and -tocopherol 60 mg kg were equally effective in ISO-treated rats. Further investigations are currently underway to find the mechanism of action and the chemical constituents responsible for the lipid-lowering property of AMLEt. Geodon for anxiety and depression

Several plans may cover your drugs, but the co-pays for covered drugs may vary from plan to plan. Most insurers are using a "tiered" co-pay system where payment varies depending on whether the drug is a generic, preferred brand-name, or nonpreferred brand-name for that insurer. A few insurers will charge you a set percent of their costs--like 25% of their discount price. The best and easiest way to get information on co-pays is through Medicare. Medicare information is available at medicare. gov or by calling 1-800-MEDICARE. They can tell you which plans cover your prescription drugs, and which plan will charge you the lowest co-pays and buy paxil. Although primarily used to treat schizophrenia and other psychotic disorders, the newer "atypical" antipsychotic agents are now used widely to treat a spectrum of psychiatric disorders, including major depression, bipolar disorder, PTSD and other anxiety disorders. While the reproductive safety of the older typical antipsychotic drugs, such as haloperidol Haldol ; and perphenazine Trilafon ; , is supported by data accumulated over the past 40 years, we have far less data on the newer atypical agents: olanzapine Zyprexa ; , risperidone Risperdal ; , quetiapine Seroquel ; , aripiprazole Abilify ; , ziprasidone Geodon ; , and clozapine Clozaril ; . Thus far, most of the data on the reproductive safety of atypical agents has been limited to manufacturers' accumulated case series and spontaneous reports, which are inherently biased: people who experience a poor outcome or an adverse event are much more likely to report having used a particular medication than people who have not experienced any problems. Among the 242 reports of olanzapine-exposed pregnancies collected by the manufacturer, there was no increase in risk of major malformations above baseline. Of 523 clozapine-exposed pregnancies, there were reported 22 "unspecified malformations." In 151 reported quetiapine-exposed pregnancies, 8 infants were observed to have congenital anomalies. Eight malformations were reported among the infants born to approximately 250 women taking risperidone; however, pregnancy outcomes were not known in many of these cases reported to the manufacturer. Taken together, these reports do not suggest an increase in the risk of major malformation above the baseline 2% to 4% risk of malformations seen in the general population, nor do they indicate any specific pattern of abnormalities among drugexposed infants. While this information has not suggested any particular concerns regarding the use of atypical antipsychotic drugs in pregnancy, we can make only limited conclusions based on this type of information.
