Submitted by Dr. John Noak Allergy season generally runs from January 1 st to December 31st. More than 60% of the population in Owyhee County have bothersome allergy symptoms during at least one period of time each year. Runny noses, congestion, itchy eyes, itchy rash, sneezing, coughing, sleepiness, dark circles under the eyes, and wheezing are some of the fun effects of allergies. A person suffering from allergies can experience one, some, or all of these symptoms. There are about a gazillion different triggers that can set off an allergy attack. Some of the more common are dust mites found in mattresses, pillows, and upholstered furniture ; , cat and bird dander dried flakes of skin ; , pollen cottonwood, sage, alfalfa, weeds, hay, etc. ; , smoke, dust, mold found in carpets and house walls ; , and farm and industrial chemicals. Allergy symptoms tend to get worse as we get older. For some people they are merely annoying, for others they are completely disabling. All is not doom and gloom. Short of locking ourselves in the house with the air conditioner going, there are many things we can do to both prevent and treat allergy symptoms. Get a set of dust mite barriers for your mattress, box springs, pillows, and comforter. The modern, coated cotton style barriers are available at discount stores like Target and Wal-Mart. They keep the dust mites in the bedding, and out of your nose. Don't try to save money by using the old plastic style barriers. They are horridly uncomfortable. Keep the cats outside chasing mice, instead of inside making the kids sneeze. If you dog is allergenic, keep it outside chasing cats. Linoleum and hardwood floors are good; carpeting is bad. Lose the dust ruffles. Jack the furniture up on blocks so you can get the mop or vacuum cleaner everywhere. When the dust and smoke start blowing it is a good idea to close the windows and change the furnace and air conditioner filters often as soon as they get dirty ; . HEPA style filters work much better. Of course, they cost more. There are several over the counter and prescription medicine options for controlling allergy symptoms. Benadryl and Chlortrimeton are effective and inexpensive antihistamines. They often have significant drowsiness as a side effect, so they aren't for everyone. Zyrtec, Allegra, Claritin, and Clarinex are new generation antihistamines. Most people respond to at least one of these medicines. They cause much less drowsiness, but are more expensive than the older antihistamines. There is hope for that stuffy runny nose. Asteli is a nonhabit forming nasal spray that opens things up pretty well. There is no drowsiness from Astelin. If your nose is stuffed up for months or years at a time an excellent option is one of the steroid cortisone ; nasal sprays such as Nasarel, Rhinocrt, Nasonex, or Flonase. They stabilize the nasal membranes to prevent histamine release. These medicines are used once or twice a day, everyday, during your own personal allergy season. Itchy eyes can be treated with Patanol eye drops. It is not a steroid, but it also works by stabilizing the membranes that cover the eye. If symptoms are severe puffy, watery eyes, itch, itch, itch, have to sleep with your mouth open, hating life then it's time to consider Kenalog. Kenalog is a long acting steroid that is given as a painless really ; shot. It knocks down the allergic symptoms for six to eight weeks. It does not cause big muscles or unwanted hair. It can help someone get through the worst of their personal allergy season. Allergy testing is spendy and allergy shots require a commitment to weekly injections for years and years. They do help most people who chose that route. Allergies suck, but we can deal with them and beconase!

We were able to use the MS Word authoring template and automatically generate both an Xml representation of our guideline, and an HTml representation, which we have deployed on our intranet. The Bedside Smoking Cessation Intervention guideline was automatically merged into the online guidelines collection. Placing it on our intranet allowed for rapid and easy access by physicians and other health care providers throughout the Kaiser Permanente Medical Care Program. This allowed it to be placed on the Kaiser Permanente intranet alongside the other guidelines, all of which are available to any computer nationwide that is connected to Kaiser Permanente's national intranet. CPGs are systematically developed statements to assist medical decision-making as to appropriate health care for specific clinical conditions3. Compliance with clinically valid and scientifically based guidelines may improve patient outcomes4. Development of CPGs without attention to implementation may result in little or no appreciable change in physician behavior or patient outcomes. Effective implementation, dissemination and ease of use of CPGs require a multi-modal approach, especially strategies utilizing evolving technology5-6. OBJECTIVES As we enter the 21st century, clinical practice guidelines are having an increasing impact on the standardization and quality of patient care. CPGs represent the state of knowledge, current at the time of their publication. With on-going changes in the state of scientific information and technology, periodic review, updating and rapid revision is necessary in order for the CPGs to be of value in patient care.
