Sometimes, staff members approach substance use issues by encouraging and pressuring tenants to stop using. Unfortunately, this usually sets up push-pull scenarios, with tenants trying to avoid staff and staff looking to "land the tenant." In some cases, individuals may want to stop using but cannot sustain sobriety and may find the staff to be just another hassle. Even though it may seem obvious that substance use is a primary cause of problems for some tenants, "hitting them on the head" with it is usually ineffective. Similarly, some people may not know "why" they drink or use substances, and it is not necessarily helpful or practical to focus on this. Instead, in an effort to build motivation for change, it is important to focus on the discrepancies between what a person wants and what that person has. Since substance use can interfere with functioning in a variety of ways, it is often helpful to address substance use as it impacts an individual's goals such as employment, reunification with family, or staying housed. The reasons to reduce or stop use may begin to outweigh the reasons to continue. The fact is, however, that substance users ultimately face giving up something that provides them with comfort in exchange for the pain of withdrawal and the loss of a familiar lifestyle. When working with people who are preparing to change, staff should help in planning to manage these losses and to avoid triggers that can lead to relapse. Individualized recovery plans are important. A recovery plan delineates the strategies an individual has decided to use to maintain abstinence as well as plans to manage urges and anticipated triggers. The plan addresses matters such as friends and support, routines and rituals, filling time, and managing feelings. This process can also be conceptualized as a "use reduction plan" for those still actively using, focusing on reducing some of the consequences of use such as disruptive behavior and poor health and luvox.

Treatment for agitated mania, but their use should be strictly short term due to the risk of dependence and poor evidence base in this setting. To date, there is little evidence to support the use of gabapentin Neurontin ; , lamotrigine Lamictal ; or aripiprazole Wbilify ; for the treatment of bipolar disorder in children and adolescents. Lamotrigine carries a black box warning from the US due to an increased risk of StevensJohnson syndrome in children under 16 years. For those who experience intolerable side effects or those who remain resistant to treatment, clozapine or electroconvulsive therapy ECT ; can be considered. SSRIs have been beneficial in bipolar depression, but the risk of a switch to a manic episode needs to be explained and families alerted to warning signs. Psychosocial treatments Psycho-education, cognitive behavioural therapy and family therapy can be considered once a child is stable on medication. Development of a therapeutic alliance with the child and family is essential and predicts greater compliance with treatment. Involvement of family members is crucial as they can assist with monitoring mood and signs of relapse. It is also important to address any factors which may lead to recurrence, such as high expressed emotion or poor communication. Support will also be needed as parents may grieve the loss of their `normal' child. Schools or other agencies may need to be integrated into the treatment programme. Maintenance treatment There is limited information regarding length of maintenance treatment. Paediatric bipolar disorder appears to carry a high risk of relapse. The risk of a relapse should be weighed against the risks of continued pharmacotherapy. Kowatch and colleagues9 recommend considering.

During the last decade, the incidence of acute rejection at one year has decreased from 40% to 20%. This coincides with the introduction of various new immunosuppressive medications. Here we report the pattern of administration of antibody induction treatment and short term outcomes. We compare IL-2 R antagonists to traditional antilymphocyte medications and to protocols using no induction in Living donor LD ; transplants. Methods: Using United Network of Organ Sharing database, all living donor transplants reported to UNOS during 1997-2000 were studied according to the patterns of administration of antibody induction treatment. Pan T cell Antibody therapies included Rabbit antithymocyte globulin Thymoglobulin ; , Antilymphocyte globulin ALG ; and OKT3. IL-2 receptor antagonists included Basiliximab and Dacluzimab. To compare outcomes, we limited the cohort to 1999-2000 to reflect current clinical practice and to control era effects. Primary outcomes included the incidence of acute rejection, renal function, and graft and patient survival at one year post-transplant. Results: The patterns of administration of various antibody induction regimens are shown in figure 1. The outcomes are shown in table 1. The utilization of antibody induction treatment has increased from 21% to 43%. There were no differences in acute rejection rates in patients receiving Pan T and IL-2 receptor antagonist compared to no induction treatment although graft survival was higher in recipients receiving IL-2R antagonists and endep.
